Goals and Learning Outcomes

People living with hypertension (HTN) and coronary artery disease (CAD) are among the highest cost members to a healthcare payer. Patients can have HTN and CAD simultaneously. Unfortunately, having one of these conditions often increases the risk of developing other types of chronic illnesses. This is especially true if HTN and CAD are not well-managed. Early detection and treatment of HTN reduces disease progression, simultaneously driving down costs. This course will provide evidence-based recommendations on providing clinically-appropriate care for HTN and CAD, teach patients self-management techniques for their conditions, work with patients to create an action plan, and identify other factors that could impact their condition. 


After taking this course, you should be able to:

  • • Indicate best practices for providing cardiac care while achieving cost effectiveness
  • • Identify markers of cardiac conditions to encourage early detection and treatment, benefiting both patients and payers.
  • • Identify different educational tools for teaching self-management to patients with cardiac conditions.
  • • Write an appropriate cardiac action plan with your patients.
  • • Identify how to provide nutritional counseling for patients with cardiac conditions.
  • • Identify potential social determinants of health that could impact adherence to the care plan.
  • Choose evidenced-based interventions for hypertension and coronary artery disease.
  • Identify different educational tools for teaching self-management to patients with cardiac conditions.
  • Recall the elements of a cardiac action plan that addresses care barriers.
