**Home Health Registered Nurse (RN)**

### Responsibilities:

* Provide skilled nursing care to patients in their homes
* Complete comprehensive patient assessments and develop individualized care plans
* Perform wound care, medication management, disease education, injections, catheter care, and other skilled nursing procedures
* Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members
* Accurately complete OASIS documentation and clinical charting
* Educate patients and caregivers on disease management, medications, safety, and preventative care
* Coordinate care with therapists, physicians, caregivers, and other healthcare professionals
* Maintain compliance with Medicare, Medicaid, ACHC, and agency standards

### Qualifications:

* Active Colorado RN license in good standing
* Current BLS/CPR certification
* Home health experience preferred, but not required
* Strong clinical assessment and critical thinking skills
* Excellent communication and organizational skills
* Reliable transportation, valid driver’s license, and auto insurance
* Ability and willingness to travel to patient homes throughout assigned territory
