Position Specifications
Case Manager - Case Management

This is a part time 0.5 FTE exempt position at our First Hill Campus in Seattle.

Responsibilities
The nurse case manager coordinates the care and service of selected patient populations through the acute care episode, across the continuum. Works collaboratively with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources and assumes a leadership role to achieve desired clinical, financial, and resource outcomes.
Qualifications
Graduate of an accredited Bachelors Nursing degree program or completion of an accredited Nursing program with a Bachelors degree or higher in a health care field. 

Current license to practice as a Registered Nurse in the State of Washington.

Must have an understanding of biological, social and behavioral theory normally acquired through the completion of a Bachelor of Nursing degree or a Bachelor degree in a health care field.  A minimum of three years registered nursing experience in an acute care hospital required. A minimum of one year of hospital case management experience is preferred.

Skills needed include excellent written/verbal communication skills in the English language, critical thinking skills, creative problem solving, proficient organization, planning and prioritization. Must be self-directed and have the ability to tolerate frequent interruption as and a demanding work load. Knowledge of funding, resources, services, clinical standards and outcomes is preferred.

Also essential is ability to employ advanced communication, negotiation, and interpersonal skills with all levels of internal and external customers, including but not limited to medical staff, patients and families, clinical personnel, support and technical staff, payors and outside agencies, and members of the community. Ability to work collaboratively with others in a complex healthcare setting.

Technical skills include ability to use a computer for clinical data entry or retrieval, demonstrated efficiency with a keyboard and mouse.  Knowledgeable in the use of general office equipment (i.e. telephones, voice mail, fax machines and photocopiers).  Demonstrated ability to use basic math skills.

Must be a mature individual capable of independent decision-making and ability to work collaboratively with others in a variety of settings and disciplines. Demonstrated ability to lead and work with a wide range of staff as part of an interdisciplinary team.

Maintains skills and competencies defined in the unit specific orientation competency skill list.

Complies with the Swedish Medical Center Personal Appearance Policy and Case Management Department professional appearance standards to include:
• Business casual attire (no jeans, stretch pants, sneakers, sweat suits, attire with decals or prominent lettering and pictures)
• Lab coats will be worn when on the patient units or when meeting with physicians and other healthcare professions, and patients and families
• No personal fragrance
