Job Description:

***Inova Fair Oaks Hospital is seeking a Stroke Program Core Measures Coordinator***

 

Provides therapeutic care in a holistic systematic way, support clinical management of patient care delivery area which produces quality patient care with optimal patient outcomes.  

 

Communicates effectively and works cooperatively with others. Has respect and understanding of the contributions of all team members. Uses integrated approach towards patient outcomes. Supports Inova's mission and strategic plan.  

 

Utilizes standards, guidelines and pathways for care delivery. Incorporates data and information to continuously improve care and practice to enhance outcomes.  

 

Ensures improvement in practice setting by assuming responsibility for self-development in life-long learning. Provides direction and guidance to others regarding practice, serves as a resource, and mentor. Leadership skill demonstrated in decision making and problem solving

Job & Education Requirements:

-Minimum of two years of healthcare supervisory experience.

Experience with stroke processes and working within a multidisciplinary process action or performance improvement team is required.

 

-Must have 16 hours in specialized stroke care

 

-Proficient English, verbal and written communication skills and computer skills (Microsoft Programs, familiar with Databases)

 

Education:

Bachelors Degree in Nursing or related field

Must have current CPR for Health Care Providers

Licensed to practice nursing in the Commonwealth of Virginia
