Registered Nurse-Transfer Center
Description
Our Transfer Center Registered Nurses transform care for our patients by evaluating and coordinating the flow process involved in taking calls, and facilitating the acceptance and triage of calls throughout the transfer call center. In this role, you will act as the primary liaison between physicians, hospitals and other care providers who seek to refer, schedule or transfer patients into the hospital.  You will provide clinical expertise, close medical communication and quality assessment for the successful referral and transfer of patients to the facility.
 
Responsibilities include:
  • Communicates with the surgery department, emergency department, nursing units, physician offices and outlying facilities to ensure appropriate placement of patients in a timely manner.
  • Evaluates, processes, tracks, documents and coordinates patient transfers for the successful referral and transfer of patients to the facility.
  • Ensures all paperwork and computer entry is accurate.
  • Conducts recorded three-way calls between referring physician and facility physician.
  • Communicates with the transferring facility to obtain accurate insurance information.
  • Ensures admissions, prioritizes/assigns beds based on appropriate level of care and patient aggregation.
  • Addresses and resolves issues arising from increased census and decreased bed availability.
  • Promotes satisfaction between the Transfer Center and all physicians, internal and external customers.

Qualifications
  • Graduate from an accredited School of Nursing. 
  • Bachelor's Degree n Nursing, preferred.
  • Current license to practice as a Registered Professional Nurse in New York State.
  • Minimum of five (5) years nursing experience, required. Two (2) years critical care experience, preferred.
  • Advanced Cardiac Life Support (ACLS), preferred.
  • Demonstrated leadership, interpersonal and problem solving skills, required.
  • Knowledge of Microsoft Office, required.
