POSITION SUMMARY
The Continuum of Care Manager is accountable to the Director of Integrated Care Management and participates in the development, implementation and ongoing monitoring of program initiatives to address the needs of Aetna Medicaid members who are receiving services along the continuum of care. Strong team in Medical Management Shared Services is looking for a new team member to continue to move Integrated Care Management and Integrated Long-Term Care Management to our health plans.

This position is fast paced and requires flexibly and adaptability to rapid changes. Critical licensure, knowledge and skills needed include;
-Independently licensed and seasoned clinicians
-Have BH and PH experience
-Ability to work with Sr. Leaders to assist in culture change.
-Helps drive program development and implementation at Medicaid health plans.
-Acts as a subject matter expert for RFP support in regards to the newly developed programs.
-Periodic out of state travel to support rolling out programs to new and existing health plans 10-25%.
EDUCATION
The highest level of education desired for candidates in this position is a Master's degree.
LICENSES AND CERTIFICATIONS
Mental Health/Licensed Clinical Social Worker (LCSW) or
Nursing/Registered Nurse (RN) is required
FUNCTIONAL EXPERIENCES
Medical Management/Medical Management - Case Management/7-10 Years
Leadership/Act as company spokesperson to external constituents/4-6 Years
Leadership/Lead a major change effort-culture change/4-6 Years
Management/Management - Product development/4-6 Years

ADDITIONAL JOB INFORMATION
-Prior experience in Managed Care/Insurance Operations required for this position.
-Must understand Integrated Care Management model -program design to implementation.
-Must be comfortable speaking in front of groups and working with Senior Leaders of the organization.
-RN- Registered Nurse, LCSW-Licensed Clinical Social Worker, LMFT: Licensed Marriage and Family Therapist, LPC: Licensed Professional Counselor, or PhD: Doctor of Philosophy (must be independently licensed) accepted.
-This position is in the corporate office in Phoenix, with a potential telework arrangement after a substantial amount of time in the office learning the programs.
