Job Description

We have an exciting opportunity to join our team as a Nurse Practitioner on the Palliative Care Service. To qualify you must have a Masters Degree in Nursing, New York State Nurse Practitioner licensure with Palliative Care Nurse Practitioner certification plus a minimum of 5 years of acute care experience. At least one year of experience as a Palliative Care NP is required.  Critical care experience as an RN or an NP is preferred. Excellent organizational and communication skills and demonstrated ability to work as part of an interdisciplinary team are a must. Flexibility with changing team goals and program development required. The successful candidate will be responsible for management of patients referred to the Palliative Care Service, functions as part of an interdisciplinary palliative care consultation service providing assessment and treatment to patients who are referred for consultation. Responsibilities include physical assessment and management of pain and other symptoms, discussion of goals of care with patients and their families, advance directives and coordination of care planning with the primary team and outside agencies.  Other responsibilities include education, both formal and informal to nursing and other disciplines, standards development, committee membership and participation in other opportunities to expand the service.
