Job Description : Academy Manor is a 170 bed skilled nursing center with long term, short term rehab and dementia residents.
We currently have an opportunity for a RN with experience with dementia residents
This is a full time benefited position days.
This is full time opportunity to develop programs to address the needs of the residents. You will work closely with the Administrator, and Nurse Management to develop and strengthen the dementia programs
POSITION SUMMARY: The 'Dementia Program' Director is responsible for administering of the 'Dementia Program' within the Center. The position requires that the individual develop and maintain a quality program which therapeutically addresses the needs of residents with dementia and their family. The role requires clinical knowledge of dementia care, leadership abilities and marketing skills.
RESPONSIBILITIES/ACCOUNTABILITIES:
1. Individual resident assessment including admission (within the scope of practice) and ongoing updates, as needed, in the home, nursing center or hospital, when necessary.
2. Coordinates, implements and monitors all care plans and other state required documents with other members of the care team.
3. Assures implementation of all policies and procedures is in compliance with local, state, federal and JCAHO guidelines.
4. Functions as a liaison during resident hospitalizations to ensure continuity of care.
5. Performs ongoing quality assurance evaluations that include quality indicators for the environment, documentation, team functioning, and all aspects of quality care provision as well as reporting identified issues back to the team, for resolution.
6. Functions as an advocate for residents and their families.
7. Provides direct services to families through communication, education and support.
8. Acts as the unit liaison for all consultants.
9. Manages all discharges/transfers from the unit.
10. Manages all research projects on the unit.
11. Oversees Program development, planning, scheduling and monitoring.
12. Acts as a role model for staff with regard to professionalism and appropriateness in resident and family interaction.
13. Interviews and makes recommendations for hiring/termination of all staff that will be working on the unit.
14. Direct and indirect supervision of all staff assigned to the 'Dementia Program' unit. Responsible for probationary and annual evaluation of all unit personnel.
15. Regular communication concerning unit functions with Director of Nursing and Administrator and other appropriate staff.
16. Directly responsible for the financial performance of the 'Dementia Program' unit. Participates in development and monitoring of unit budget.
17. Maintains appropriate records, correspondence and documentation for management of the unit.
18. Develops and implements new systems as new policies are developed.
19. Implements ongoing team building efforts.
20. Responsible for the majority of phone inquiries regarding the 'Dementia Program'.
21. Liaison with all departments in center.
22. Provides orientation as well as ongoing in-services on dementia care to staff, including sensitivity training and mandatory 'Dementia Program' training.
23. Provides family education through support groups, family council, and other methods of information such as newsletters, brochures, etc.
24. Provides community education through lectures before groups and organizations within the community.
25. Participation in committees within the center and the community that are dedicated to dementia care.
26. Supervises appropriate student internships.
27. Develops and implements yearly marketing plan.
28. Markets the Program through public speaking, committee participation, community partnerships to be viewed as a dementia specialist in respective community.
29. Monitors for changes in the market and competition.
30. Performs other duties as assigned.

Qualifications : SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:
1. Massachusetts Nursing License
2. 2 to 3 year experience working with dementia residents
3. Two years of supervisory experience in an appropriate healthcare setting, or approval by the Administrator and Area Director of Dementia Services/Care.
