CASE MANAGER Tracking Code 6229 Job Description The goal of the Case Manager is to ensure delivery of optimal care while promoting fiscal responsibility. Ensures the daily operations, procedures and standards of care are maintained in accordance with hospital and county guidelines. Participates in the coordination and direction of care for patients from admission through discharge. The Case Manager must be prepared to address the issues of health assessment and appropriate intervention. In addition, evaluates available resources, plans and implements necessary services and monitors outcomes. The role requires the nurse to have an understanding of financial reimbursements and third party payments. Acts as a resource to ancillary and nursing staff. Coordinates and promotes an advanced level of nursing care for complex, critically ill patients. Utilizes critical thinking, interpersonal and technical skills within the patient care continuum. Participates in data collection as required. Basic Qualifications:Current California RN license. Minimum Qualifications:One year Case Management experience required. BLS for Health Care Provider from the American Heart Association required by start date. Preferred Qualifications:Bachelor's degree in nursing strongly encouraged.
