Provides home care visits and assumes on-going primary responsibility for an assigned caseload of patients within a designated area. Responsible for submitting documentation of patient visits in a timely and accurate manner. Communicates pertinent findings to case management coordinator. Required to work one weekend per month and designated holidays.
Job Requirements:
Associates degree or Diploma from an accredited school of Nursing; Bachelors Degree preferred. Minimum of one-year home care experience required, preferably in a CHHA or LT Home Care program. Licensed with current Registration to practice as Registered Professional Nurse or Licensed Practical Nurse in NYS. Ability to work within an integrated health care team. Demonstrates effective critical thinking and problem solving skills and able to work independently. Ability to communicate in English both verbally and in writing. Proficiency in interviewing and physical assessment skills. Word and Excel knowledge preferred.
