Case Manager (S129)

General Summary: Implements case management activities that promote continuous improvement in the coordination of care; monitors and manages clinical resource consumption; ensures clinical documentation is accurate; provides continuity of care and post hospital planning in order to optimize cost efficient resource utilization and maximum reimbursement. Supports the Mission of Trinity Health and Holy Cross Hospital. 

Minimum Requirements: RN with BSN (other clinical degree as appropriate may be considered); Master Degree preferred. Certification in Case Management or Continuing Care preferred. Licensed by State of Maryland. At least three years acute care experience required; Utilization/Quality Management, Discharge Planning and/or Clinical Documentation enhancement experience preferred. Knowledge of overall hospital operations, especially in areas of reimbursement and regulatory matters. Highly articulate with strong written and verbal communication skills. Computer literacy and analytic skills required. Able to communicate effectively, demonstrating good audio and visual acutiy. Demonstrated ability to work with all age group of patients.
