MANAGER, CARE COORDINATION DEPARTMENT

The Health Plan of San Mateo (HPSM), a managed care health plan, seeks a Manager to plan, organize, develop, and manage the work of the Care Coordination Department.   Primary duties include the following:

  • Manage licensed and unlicensed staff to maintain and continuously develop a care coordination program focused on ensuring high risk members receive timely and appropriate health services, including post-hospitalization and post-ED stabilization services, rehabilitation, transitional care and skilled nursing services, specialist services, home care services, comprehensive assessments, and care planning.
  • Supervise the work of a team of nine staff, including: Nurse Case Managers, Social Services Case Managers, and Care Coordination Technicians.  
  • Research available internal and external resources.  Serve as a liaison/coordinator between HPSM and San Mateo County community agencies, as well as between staff and departments within HPSM in order to match needs to resources.
  • Continuously develop and update policies, procedures, and standards for the care coordination program.
  • Establish and maintain collaborative working relationships with internal and external entities to ensure collaborative planning, resolution of problems, and open communication.


Requirements
Licenses and Certificate: A valid California license as a Registered Nurse. Certified Case Manager is preferred.
Education and experience equivalent to: A Bachelor’s degree in nursing or a related field.  Five years of progressive managed care experience in the area of utilization management, case management, and coordination of care with community resources.
Knowledge of: Program development and implementation; regulatory compliance, Centers for Medicare and Medicaid Services (CMS) regulations, and Medicare and Medi-Cal programs; and care management and the desire and passion to improve healthcare outcomes for HPSM members.
Ability to: Supervise and mentor staff, analyze situations independently and make appropriate decisions; provide excellent customer service; establish and maintain effective interpersonal relationships with all levels of staff, other programs, agencies and the general public; coordinate/negotiate complex treatment plans with stakeholders; prepare written reports and maintain accurate records; communicate effectively, both verbally and in writing, with individuals from varying cultural and ethnic backgrounds; and effectively utilize computer and appropriate software (Word, Excel, Access and Outlook).
Other requirements: Must have own vehicle and valid driver’s license with proof of insurance in conformity with state law minimums.
