Nurse Manager – Care Management

As part of the newly-created state-wide initiative of Health Home Care Coordination, where CHN has been designated as a Lead Organization in this endeavor, we are seeking experienced, highly-motivated, and creative individuals with demonstrated supervisory skills and a strong customer service orientation to serve our customers including patients, caregivers, employees, candidates, vendors, and members of the community as the Nurse Manager – Care Management.

The Nurse Manager – Care Management is responsible for the care management of high risk chronic illness patients to promote effective education, self-management support, and timely healthcare delivery in order to achieve optimal clinical outcomes. Additionally, the Nurse Manager – Care Management is responsible for the supervision of staff engaged in care coordination and community outreach activities, and developing community linkages in order to enhance service delivery.

Skills

The successful candidate for this position will minimally possess a Bachelors Degree in Nursing(BSN).   Additionally, the successful candidate will have demonstrated supervisory skills and care management experience, excellent communication skills, a strong customer service orientation, the ability to work in a team environment, and be flexible in taking on various projects.
