~CB~ Position Summary: Coordinates home care services in the acute setting by utilizing the nursing process. Assures smooth transition of patient(s) from the hospital to the home environment. Works with physician to provide home care services to prevent unnecessary hospitalizations. Recruitment Requirements: - Graduate from an accredited school of nursing.
- Associates degree
- 1-2 years of community health experience.
- Recent acute care experience.
- Possess a current NJ RN or LPN license
- CPR certification required.
- Possess a high level of analytical ability, detail oriented, strong interpersonal skills, communication skills. Ability to prioritize multiple demands with use of organizational skills and marketing skills.
- Able to travel to patient's home within service area.
- Valid driver's license.
- Able to demonstrate professional procedures in the home care setting.
- Able to use office equipment; i.e. computer, fax, copier, (generalized equipment),beeper, car phone.
- Ability to work flexible hours with weekends and holidays included.
