| Occupational Summary:Work with a physician led critical care service (CCS) to provide medical services to inpatients in need of critical or acute care at Duke Raleigh Hospital. Such services will include the collection and documentation of clinical and administrative data, performance of diagnostic and therapeutic procedures, ordering of laboratory studies, ordering and provision of professional consultations, and the personal provision of direct patient care. Work Performed:Perform diagnostic and therapeutic procedures to assist physician in diagnosing medical and surgical conditions and prescribing and/or coordinating necessary treatment and services for quality patient care. Collect, collate and record normal and abnormal data for review by the physician and the care team. Taking patient histories, conduct physical examinations, and record the data in the patients' medical record; complete intake and daily progress notes; prepare patient workups and summaries noting pertinent positive and negative findings; obtain psychosocial history relevant to patient care; develop and maintain therapeutic relationships with patients and families. Order laboratory studies and diagnostic procedures in conjunction with the CCS physician(s); explain indications, preparation, nature and anticipated effects including potential adverse events and risk of scheduled diagnostic and therapeutic procedures to the patient and/or their representative; prepare and obtain procedural informed consent documents in conjunction with CCS physician(s). Carry out therapeutic procedures including but not limited to: Endotracheal Intubation Initiation and management of invasive and non-invasive mechanical ventilation Placement of central venous, pulmonary artery (Swan-Ganz) and arterial intravascular catheters Placement of CVVHD catheters Urgent/Emergent chest decompression including chest tube placement Diagnostic Lumbar Puncture Diagnostic Ultrasound appropriate to the ICU environment (vascular access, bladder scans, guided thoracentesis, evaluation for intra-abdominal fluid, limited cardiac exam, etc.) General wound care (dressing changes, removal of sutures and drains) Non-surgical placement of enteral feeding tubes Monitor patient progress in accordance with a diagnostic/therapeutic plan developed in conjunction with the critical care physician(s) and other team members. Communicate progression and/or need for plan revision with the attending critical care physician on a daily or more frequent basis as needed. Document suchprogress and communication in the medical record. Assure the common understanding of the plan of care among all critical care and supporting service members and facilitate communication between team members. Collaborate with critical care physicians and other team members to identify, document, adopt and monitor compliance with relevant critical care Clinical Practice Guidelines (CPGs) where evidence-based CPGs exist. Provide initial consultative services to other inpatient areas of Duke Raleigh Hospital where high acuity patients are admitted or where transition to higher acuity care needs occur (PACU, PCU, General Care Floors, ED, etc.) and respond to clinical emergency condition activations (Rapid Response Team, Code "Blue") as a representative of the CCS. Where ICU services appear to be required following such consultations or events,the practitioner will be expected to communicate with the attending critical care physician in the process of implementing the provision of subsequent CCS involvement. Participate in quality improvement activities of the CCS. Participate in other administrative activities of the service including but not limited to participation in medical and hospital staff committees as representatives of the CCS, the development and promulgation of protocols, standing orders, and other service documents in paper or electronic form, and the definition, organization, collection, and regular reporting of internal CCS quality and service indicators. Participate in academic and research endeavors of the service to include conduct of clinical research, independent collaboration within the Duke University system where consistent with Duke Raleigh CCS service goals and obligations and teaching of professional students as may be requested. Participate and support the community involvement of Duke Raleigh Hospital as it relates to initiatives, goals, and services common to the enterprise and the CCS. Education Completion of an accredited Nurse Practitioner program. A Bachelor's degree and Master's degree in Nursing required; Critical Care / Acute Care NP master's or completion of ACNP/CCNP post master program strongly preferred.Experience |
