Wonderful opportunities in our Behavioral Health & Primary Care Integration Program!

This new program's integrated and coordinated design includes primary care and behavioral health care professionals working together to meet consumer's health care needs through a team approach.

Positions needed:

FT Primary Care Adult Nurse Practitioner to provide primary medical health care services to our consumers.  Assess & diagnose health conditions, conduct health exams, prescribe medications, other duties as assigned.  MSN degree, NJ RN, NJ APN, DEA & CDS, ANCC cert., CPR cert., Preferred: 1 yr. exp. working with psychiatric population.

FT RN to conduct health exams, complete intakes and assessments, monitor health status, participate in the development of the recovery plan, other duties as assigned. NJ RN, CPR cert., Preferred: BSN + 1 yr. exp. working with psychiatric population.

PT Diabetes Educator to educate consumers on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing.  Collects consumer's data to monitor health status and proactively manage the consumer's condition.  BA & at least 2-4 yrs. of exp. in the field or related area, current certification as a Certified Diabetes Educator.  Ideal candidate will possess: RN or RD with 4 year degree.
