Job Description: 
The Home Health Specialist RN Case Manager position is responsible for coordinating the delivery of all care/services appropriate to the needs of the patients of all ages and their families.  Complete a thorough physical and psychosocial assessment of the patient and identify effective interventions with the patient, family under the direction of physician to address the specific care goals.  Maintain complete and timely records, verify authorizations.  Provide clinical supervision as appropriate.
 
Job Qualifications: 
MN Nursing License and current CPR (BLS for Health Care Providers) Certification Required.  Minimum of one year recent RN experience in acute care, home care/hospice or long term care. Two plus years RN experience in med/surg, critical care, oncology or high tech home care/hospice preferred.  Basic computer skills, reliable transportation, valid MN driver's license (or license that matches your current permanent residence), current and adequate auto insurance.
