Job Description 

Responsibilities Maternal/Child Case Manager include:

  • Providing cost effective, coordinated care for pregnant and recently delivered members
  • Collaborating with the high risk members' OB/GYN care provider on care planning and referrals to medical and behavioral health specialists
  • Conducting in depth risk assessments to confirm member need for case management
  • Providing education, referral, advocacy, and coordination regarding agencies and programs within the members' local community
Required Skills 
  • RN
  • 5+ years Ob/GYN or Maternal/Child clinical experience, or equivalent staff nurse experience
