This position provides utilization management by monitoring the medical necessity and the appropriateness of the level of care through conducting admission and continuing stay reviews, and in processing discharges from the hospital. This includes interviewing patients and families, assembling the necessary paperwork, communicating with post-discharge providers. The position improves the outcomes for patients by coordinating the interdisciplinary team to provide appropriate services throughout the continuum of care. To focus on the entire episode of illness, crossing all setting in which the patient receives care through discharge to ambulatory services, home health, skilled nursing, long term care, or home as indicated. Access to all Patient Health Information for treatment. Qualifications Current California RN License. BSN Four years experience as an RN preferred National specialty certification preferred
