I. POSITION SUMMARY

Position Summary: Functions as a member of patient care team in providing competent

care to patients according to their individual needs. Coordinates

patient care and delegates responsibilities to team members within

their scope of practice and competencies.

II. POSITION REQUIREMENTS

A. Licensure/ Certification/ Registration: 1. Valid California RN license.2. Current BCLS certification.3.

National RN Certification preferred.

B. Education: 1. High school diploma or GED.2.Graduate from an accredited

school of nursing.

C. Experience: 1. Minimum of one year acute care hospital experience in area of

specialty.

D. Special Qualifications: 1. Computer literate.2. Competencies are reflected by valid

California RN license and are validated during preceptor/orientation

process.

B. Key Accountabilities / Competencies

Department Proficiency:

1. Develops realistic discharge plan based on self-care abilities, available resources, cultural considerations,

and age/developmental factors.

2. Patient Teaching - Assesses and prioritizes patients' educational needs and provides patient education to

improve patient's/family's understanding of health status, increase patient participation in care decisions,

and maximize patient self-care skills.

3. An admission educational assessment is completely and accurately documented for each patient.

4. Educational activities are structured around any factors that may impact learning.

5. Patient education/instructions are documented on the interdisciplinary patient education record and

communicated to other disciplines so teaching is consistent and complementary.

6. When appropriate, an interactive approach is used.

7. Communicates information to patient care team and patient/significant others in a timely, accurate, and

clear/concise manner.

8. Communicates all changes in patient status to charge nurse immediately.

9. Utilizes established channels of communication. Demonstrates Customer Service behaviors by courteous,

professional behavior in all interactions with personnel, medical staff, and other departments to promote

an environment of collaboration in order to meet unit and hospital objectives.

10. Organizes work to meet high priority patient needs first.

11. Anticipates patients' needs (frequent rounds, bathroom schedules, answers call lights promptly).

12. Offers help to others without being asked.

13. Completes assignment within the specified time frame.

14. All documentation must be accurate, complete and legible.

15. Transcribes and notes orders within 2 hours.

16. Documentation reflects time of occurrence.

17. Discharged patient's record must be completed and disassembled (includes all MAR's and old records).

18. All hospital-required reports (i.e., transfer reports, fire drill reports, RIR's, Statement of Concern, Blood

Glucose Accession Record), are completed within the shift of occurrence.

21. Scan new orders in a timely manner.

29. Complete all mandatory requirements within probationary period or specified time frame without

reminders.

30. Participate in unit responsibilities:Unit Projects, PI/QA (crash cart, glucose meter QC, medication

refrigerator log), unit meetings, staff education, cleanliness, count narcotics.

31. Arrive to work on time.

32. Age-Related Competencies - Demonstrates the knowledge and skills necessary to provide care appropriate

to the age of the patients served, including knowledge of the principles of growth and development and

psychosocial characteristics and interventions for Adolescents to Geriatrics.

33. Age-Related Competencies - Demonstrates the ability to assess and interpret age appropriate data about

the patient's status in order to identify age-specific needs and provide the care needed.

34. Age-Related Competencies - Demonstrates age appropriate communication skills for the patient

population served.

35. Assessments and reassessments are performed often enough to ensure knowledge of the patients' current

condition, response to care, and to prevent unnecessary complications.

36. Assessments are initiated within one hour and completed within 8 hours of admission.

37. Assessments are complete and contain accurate information which includes: Significant history, physical

findings, psychosocial assessment, growth and development assessment, assessment of educational needs,

information pertinent to discharge planning, and initial screening for appropriate referrals to other

disciplines.

38. Planning - Interprets findings and develops an individualized plan of care which documents prioritization

of and coordination of care to meet assessed immediate and ongoing patients needs.

39. Care plans reflect realistic and measurable goals and show collaboration of and coordination of disciplines.

40. Develops interventions to achieve patient goals.

41. Regularly reviews and revises plan of care based on patient's progress and response to treatments and

interventions.

42. Completes interventions that are not pertinent to the patient's needs or a current priority for the patient.

43. Implementation - Provides (or makes provision for) safe, competent patient care that meets patient needs,

the medical plan of care, and the patient plan of care.

44. Delegates/assigns patient care activities based on patient needs and the caregivers'

abilities/competence/scope of practice.

45. Directs and coordinates unit/dept. activities to ensure patient care is not compromised.

46. Acts as a resource to staff by providing information, problem solving to immediately resolve patient care

problems.

47. Identifies and responds to emergency/critical situations and directs others to ensure effective response.

48. Acts as a patient advocate to ensure patient needs are met.

49. Evaluation - Evaluates patient care/patient response and updates/modifies plan of care as changes in

patients' condition occurs to ensure patient goals are met.

50. Keeps patient, staff, and other disciplines aware of changes in patients condition/plan of care.

51. Identifies and resolves gaps in service.

52. Accurately documents interventions, outcomes, patient care, and patient response.

1. Solves customer problems, treats customer with dignity and respect, and acknowledges customer and

responds quickly to customer needs.

1. Ethics and Compliance - Conduct demonstrates compliance with and reporting of suspected violations

of all relevant laws, regulations, RCH's Compliance Policies and Procedures, and Corporate's CODE OF
