Providence is calling a Licensed Nurse. We will consider an LPN or RN. We are seeking Licensed Nurse for its Long-term Care Unit who will be responsible for planning, organizing, direct, and evaluates nursing services as part of the Interdisciplinary Team. In collaboration with others, administer an individualized, comprehensive and resident/patient centered Plan of Care.
Key accountabilities assigned to this position will include, but will not be limited to:
Accountability and decision-making for patient care outcomes.
Plan, direct, supervise, evaluate, and assist in the nursing care of h is/her caseload of patients.
Contribute to the total plan of care for the patient by making pertinent nursing observations and participating in multi-disciplinary team care conference as needed.
Maintain regular and ongoing communications with the patients, family, nursing staff, interdisciplinary team members and others to ensure 24-hour continuity of care and unified approach by the team for each patient.
Maintain knowledge of current clinical condition of all patients under his/her jurisdiction.
Routinely check patient rooms flr cleanliness, infection control and neatness; each patient for appropriate grooming, hygiene and apparel; and that NACs duties are being performed as directed.
Maintain appropriate documentation and legal clinical records of patients.
Participate in survey inspections by authorized government agencies including completion and delivery of any requested correspondence.
In coordination with the nurse manager, coordinate nursing assignments and dlegate aspects of nursing care to NA to assure continuity of care, and assist in the orientation and development of new employees on the team.
Participate in new admit process.

Position Specifics. This position is located at Providence Providence Mother Joseph Care Center in Olympia, WA. This is a Full-time Evening Shift opportunity.
REQUIREMENTS Education:Must have graduated from an accredited nursing program Licenses/Certifications:Must have a WA State LPN or RN license; current CPR certification. Experience:Prefer candidates with one year of nursing experience, preferably from a long-term care environment; will consider a new graduate.
