Responsibilities
Under the supervision of the site manager, Department of Care Coordination, the care coordinator is responsible for coordinating and integrating high quality, cost effective care delivered to patients across an episode of illness or throughout the entire continuum of health delivery. RN with BSN or other equivalent degree, Master degree preferred. Five year's clinical experience. Two years care management in an inpatient, HMO, Insurance or other outpatient setting preferred. Five years of health care experience in an acute care setting required. One year of management experience preferred. Basic computer skills. Knowledge of Microsoft Work, Access, and Excel preferred. Current NYS licensure. Strong organizational and leadership skills. Excellent interpersonal skills. Role model and change agent behavior. Ability to problem solves and prioritizes, ability to function as core member of an interdisciplinary team. Strong understanding of the business of health care.
