Job Description:
Assumes on-going primary responsibility for an assigned caseload of patients in order to coordinate, implement and evaluate patient plan of care in the patient's home. Coordinates activities with other health team members to ensure optimal patient care and communication of pertinent findings. Reviews patient insurance information, identifies appropriate billing source, and bills appropriate insurance based on patient needs.

Job Requirements:
Associates/Diploma from an accredited school of Nursing, Bachelors Degree preferred. Minimum of one year acute, medical-surgical nursing experience. CHHA/LTHHCP experience preferred. Licensed with current Registration to practice as Registered Professional Nurse in NYS. Excellent clinical and assessment skills with the ability to work within an integrated health care team. Effective critical thinking skills and the ability to problem solve independently. Must possess excellent verbal and written English skills. This is a field position requiring travel in a designated area within NYC.
