Job Details:

  • The Facility-based Hospice Home Care Nurse reports to the Team Manager and works a PRN schedule. He/she is responsible for identifying patient/family care needs, and for providing exceptional nursing and supportive care for our terminally ill patients and their families. This position requires a guarantee of eight shifts per month serving clients in our Baltimore City/County assisted living and skilled nursing facilities.

    Education

    Diploma, ADN and/or BSN from an accredited institution

    Experience

    Two years of nursing. Home Health or Hospice experience preferred but not required.

    Skills

    ·        Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection

    ·        Strong time management skills

    ·        Excellent interpersonal and communication skills

    ·        Ability to be patient and family focused

    ·        Skill in the use of EMR (Electronic Medical Record) and/or ability to learn

    Licensures, Certifications

    ·        Current MD nursing license
