SUMMARY:
The Case Manager is responsible to implement the plan of care for specific patients, beginning on a pre-admission basis and following through placement and into follow-up/after care and is empowered to facilitate mid course care changes, in conjunction with the patient care team. The Case Manager collaborates with the patient, family, Interdisciplinary Care Team members and external healthcare providers to assure safe, timely and efficient care.
REQUIREMENTS:
Graduate of an accredited school of nursing. BSN or Bachelors degree in Health Care Related Field is required or enrollment in a BSN program required with case management experience. Masters Degree in Healthcare Administration, Nursing or Business Administration is preferred. Minimum of 5 years of relevant clinical experience required, of which there must have been demonstrated abilities in the areas of discharge planning, utilization and quality management. Prior case management experience highly desirable.Must maintain current Maryland License as an RN. Must maintain current CPR certification.
