The Speech Language Pathologist is responsible for diagnosing, evaluating, screening and treating adults and children with speech/language/cognitive/swallowing disorders and cognitive disorders; generating all required reports and documentation procedures and maintaining liaison with referring physicians. He/She demonstrates competency in caring for patients of specific ages as designated in unit structure standards.

• Completes, assesses patient's needs prior to establishing the plan of care.
• Provides direct patient care on a daily basis or as indicated by patient need/condition and in the case of a staffing shortage, according to established priority guidelines.
• Assesses patient's progress and adjusts treatment plan as indicated.
• Consults with appropriate medical personnel regarding patient's needs, status, and discharge plans. Maintains accurate and current documentation in patient's medical record.
• Provides instruction and teaching of therapeutic procedures and ongoing routine care as needed.
• Maintains accurate patient treatment records and completes billing from billing sheets according to department billing procedures and within established timeframes.

Qualifications:

• Relevant Master's Degree
• Speech Pathology License from AZ Dept of Health
• ASHA Certification required
• Current BCLS
• 1 - 2 Years experience
