Summary of Essential Job Duties:
Responsible for the planning and coordination of all aspects of the work up and care for the advanced heart failure (AHF) and ventricular assist device (VAD) patients throughout the continuum of services – from initial referral to follow-up care. The Nurse Practitioner (NP) is responsible and accountable for the application of the nursing process, supervision of clinical practice, research involvement, consultative services, inter and intradepartmental liaison, education, leadership and patient care management across the health care continuum from preventative and acute care through discharge in addition to outpatient patient management. The NP is a member of the VAD team which provides 24x7 coverage for any VAD patients that live in the community with the device. The AFH NP serves as a patient advocate throughout the care process, and represents the medical center at regional and national meetings/conferences.
Education and Work Experience Preferences: Master of Nursing Degree required. Current State of California Registered Nurse and Nurse Practitioner License. Graduate of an accredited Nurse practitioner program approved by the California Board of Registered Nursing as defined in Section 1482, Nurse Practice Act, Business and Professional Code. National Certification by respective accredited professional nursing organization such as the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners (AANP), or Neonatal Academy of Nurse Practitioners (NANP). New nurse practitioner graduates who are eligible for national certification will be allowed a period of one year from the date of their training program graduation to obtain national certification. Curent NP Furnishing License/DEA. Current certification in Advanced Cardiac Life Support (ACLS), Neonatal Resuscitative Program Certificate (NRP), specific to Neonatal NP's, and/or Pediatric Advanced Life Support (PALS), specific to coach NP's. Demonstrates a commitment to customer service and an ability to meet the needs and expectations of patients and healthcare colleagues.
Experience:
Full command (verbal and written) of the English language.
Minimum of 2 (two) years acute hospital experience.
Physical Demands:
Perceptive acuity and physical ability to perform all direct and indirect patient care functions including walking, sitting, standing, bending and light lifting required.
