This position will require the Nurse practitioner to manage patient caseload across their residential group homes in Manhattan, Brooklyn and Bronx.
The responsibilities of the NPs selected for this position are:
- Utilizing the diagnosis and prognostic indicators to ensure that the patients meet the medical criteria for admission to the hospice program
- Creating written plans of care with the active collaboration of an interdisciplinary team
- Providing ongoing education and consultation on medical issues
- Maintaining regulatory compliance
