Position Summary:
Facilitates the flow of patient care throughout the acute hospitalization, utilizing the care management process. Maintains high quality and efficient service delivery. Functions as a patient advocate and liaison to family, physicians, other healthcare teams members, community resources and third party payers. Provides necessary appropriate interventions that foster independence and wellness, and prevent inappropriate unnecessary admissions and/or utilization of resources.
Age specific criteria listed is to assure individual performance demonstrates employee knowledge and skills necessary to provide psycho-social, emotional and educational needs appropriate to the age of the patients served in his/her assignment. The skill and knowledge needed to provide such care might be gained through education, training or experience.
Recruitment Requirements:
Licensed Registered Nurse/ Degreed Social Worker or Bachelors Degree in pertinent Healthcare related field
Minimum 2 years medical/surgical or home health experience
Working knowledge of managed care and third party reimbursement
Experience with case management, evaluation/assessment of clients
Basic computer skills required
Advocacy and problem solving skills
Knowledgeable of community resources, including availability of services
Excellent time management and written and verbal communication
Possesses strong organizational and negotiation skills
Possesses strong team work skills
