The Case Manager is a Nurse with strong clinical knowledge and leadership skills. The Case Manager collaborates with the patient, family, physician, interdisciplinary team, and payer representative to assess, plan, implement, coordinate, monitor and evaluate options and services to promote optimal patient outcomes. The Case Manager monitors the provision of cost-effective care to ensure care is given in the most clinically appropriate setting. The Case Manager facilitates compliance with regulatory requirements and organizational performance standards.

Qualifications
EDUCATION:
Minimum: Graduate of an accredited school Nursing , Associate's Degree in Science, or other qualification recognized by the State of Florida for RN licensure.
Preferred: Bachelor's Degree
EXPERIENCE:
Three (3) years bedside nursing experience in the relevant clinical specialty
REQUIRED CERTIFICATION/LICENSURE/REGISTRATION:
Florida RN license; BLS
OTHER QUALIFICATIONS:
Conversant knowledge and experience with case management care coordination, and/or utilization review.
Track record and comfort in collaborating with physicians.
Ability to lead and coordinate activities of a diverse group of people.
Demonstrated ability in critical thinking, problem solving and communications.
Basic skills with computers including word and excel
