Functions as the key member of the health care team responsible for ensuring efficient delivery of quality care, resource utilization, and service levels as related to patient care services. Specific role functions include the facilitation of timely and appropriate discharge planning, facilitating the psycho-social needs of all patients regardless of location, evaluation and referral of patients post-hospitalization (including transfer to another facility, resource finding, etc.), collaboration and communication of plans of care to the physician, nursing staff, patient and/or significant other and to other members of the multi-disciplinary team as appropriate, identification of inappropriate or untimely services, identification of opportunities for improvement, and ensuring compliance with regulatory standards. Active participant of the Hospital-wide Performance Improvement Committee.

Qualifications
Qualifications/Requirements

Education: Graduate as an RN, LPN.

Required licensures/certifications
Current RN or LPN FL Licence.
         
Experience. Knowledge in managing various medical/surgical case types, reimbursement methodologies, DRG optimization, resource utilization, performance improvement tools and techniques preferred. Ability to effectively communicate. Proficiency with Interqual Criteria and hospital case management experience preferred
