The Clinical Program Manager for Heart Hospital Education is responsible to provide specialized nursing care to a select group of patients, and coordinate and manage major initiatives and clinical projects in the designated Clinical Program. The Clinical Program Manager role models the mission of the Clinical Specialty program at all times; to promote a culture of patient-centered care that emphasizes service and compassion. He/She interfaces directly with physician and clinical leaders of the Mount Sinai Medical Center, Departments Heads and all levels of staff | Responsible To | Nursing and/or Medical Director | Responsible For | Development, implementation, evaluation and overall management of the specified clinical program and major initiatives that fulfill the mission of specified clinical programs Clinical Practice: Participates in the development, implementation and coordination of clinical services and programs for designated patient populations. Activities may include but are not limited to: Patient screening Treatments and procedures Patient and family education Discharge planning and follow-up Case management Research Interdisciplinary teamwork
