Program Summary

 

The AtlanticCare ACO is a healthcare organization characterized by a payment and care delivery model that seeks to tie provider reimbursements to quality metrics and reductions in the total cost of care for an assigned population of patients. The Valley Hospital is an active part of this organization. One of the many patient populations TVH is currently focused on is the pulmonary disorder population. By creating a Pulmonary Center, we hope to meet the needs of this population.

Valley Health System’s Pulmonary Center will provide comprehensive state of the art

pulmonary services utilizing a multidisciplinary approach. Spearheading this program, a Pulmonary Program Advanced Practice Nurse (APN) will provide patients with individualized attention and support, and will be available throughout the patient’s healthcare journey. Combined with the expert medical care from our board-certified physicians and our specialized team, the APN will support physicians as they manage their pulmonary patients during hospitalizations and facilitate the continuum of care by connecting the patients with appropriate services and healthcare providers in their community.

 

APN Pulmonary Center Role

 

v  Assists in development, implementation and evaluation of the goals and objectives for the designated area. 

v  Coordinates and develops programs and services to ensure the needs of the program are continuously addressed.

v   Participates in special programs (World COPD day, Great American Smoke out, etc).

v  Develops appropriate tracking system and maintains statistics generating monthly and year end reports.  Works with IT department and other departments to ensure accurate and efficient collection of data.

v  Establishes the standard of care for the delivery of seamless clinical services for all patients and families.

v  Collaborates with physicians, nurses, and other key personnel to provide multidisciplinary approach to meet patient needs. 

v  Coordinates and participates in site specific committees, and other patient care rounds as appropriate.

v  Serves as liaison to and collaborates with community organizations.  Establishes and maintains contacts with community based organizations providing education, information and services as appropriate.  Participates/attends events and/or initiatives offered by these organizations as needed or assigned. Ensures information about these organizations and their services are made available to patients and staff.

v  Collaborates and consults with other departments to standardize best practices, share information and collaborate on joint initiatives as necessary. 

v  Interfaces with physicians, nurses, managers, and others to promote areas of expertise and integrate relevant service into appropriate areas,

v  Develop and maintain pulmonary center website. Review all brochures and handouts of the respiratory department as well as those directly related to the Pulmonary Center.  Provide patients with brochures/handouts as necessary.
