Director of Facility Based Care (RN)

A healthcare facility is seeking a professional, organized, and skilled Registered Nurse (RN) to join its team as the Director of Facility Based Care. The RN who fills this role will be responsible for the planning, management and coordination of the clinical operation of the delivery of hospice services in the nursing homes, inpatient units, and swing contract beds for the hospital. Healthcare Facility 

As the Director of Facility Based Care (RN), you will:

• Plan, manage, and coordinate the clinical operation of the delivery of hospice services in the nursing homes, inpatient units, and swing contract beds for the hospital
• Be responsible for education oversight to ensure the efficient provision of effective, high quality services in compliance with Federal, State and local regulations and JCAHO standards

• MSN
• 10 years of experience as an RN
• 5 years of leadership experience in hospice
• Excellent problem solving and interpersonal skills    $

• CHPN certification
