Job Details:
  • The Triage RN reports to Associate Clinical Director. He/she is responsible for triaging patient and caregiver telephone calls, assisting in the solution, palliation of symptoms and provision of education. Assists with identifying patient/family care needs and for providing nursing and supportive care for the terminally ill patient and their families, in conjunction with case managers and physicians. Maintains relationships with families and patients. Provides support to care team members by acting as a liaison/coordinator with physician offices, insurance case managers, pharmacies and DME company.

    Education

    Diploma, ADN and/or BSN from an accredited institution

    Experience

    One to two years of nursing. Home Health or Hospice experience preferred.

    ·   Skill in the use of EMR (Electronic Medical Record) and/or ability to learn

    ·   Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection

    ·   Ability to perform consistently in stressful situations.

    ·   Strong and creative, problem-solving skills with the ability to prioritize and delegate.

    ·   Ability to simultaneously handle multiple assignments and projects with attention to details, efficiency and accuracy and deadlines.

    Licensures, Certifications

    ·   Current Maryland State license as a Registered Nurse

    ·   Certification in end-of-life care by the NBHPNA within three years of employment

    Physical Requirements

    ·   Ability to concentrate and pay close attention to detail often in stressful situations.

    ·   Ability to perform home care visits Generally sedentary work but may require standing and walking for approximately 10% of work time
