Job Description:
Clinical Resource Manager - Case Management.
Clinical Resource Manager (CRM) collaborates with patients by assessing, facilitating, planning and advocating for health needs on an individual basis; facilitates communication and coordination between all members of healthcare team involving the patient and family in the decision making process to minimize fragmentation of healthcare; educates to facilitate informed decision is link between individual, hospital, payor and the community; encourages appropriate use of medical facilities and services; improve quality of care and maintain cost effectiveness on a case by case basis; is an advocate for patient and hospital.
Experience:
Minimum of three years medical/surgical or critical care experience required. BLS Certificate Required. Current State of Florida RN license required
