General SummaryCoordinates patient care with patients/significant others and themulti-disciplinary care team and collects and evluates information to facilitate care by: Assuring admissions continued stays and ancillary services are medically necessary cost-effective and provided in the appropriate setting to qualify for reimbursement. Intervening as needed to assure that care is coordinated and timesly. Performing early identification of discharge plans in doing home health referrals and durable medical equipment (DME) referring cases needing psychosocial assessment and intervention to Social Services.
Minimum Job Qualifications Minimum Education: Two Year Degree-ADN
Minimum years in field previous to employment:2- 3
Minimum Job Qualifications:BSN preferred.
Experience should include 3-5 years of clinical practice with experience acquired in assigned specialty.
Prior experience in Quality/Utilization Management and/or Discharge Planning is preferred.
Case management certification or willing to certify is encouraged.
