Registered Nurse (RN) - Home Care California

The VITAS Nurse is a member of the interdisciplinary team and is the pivotal person in identifying the physical, psychological, social and spiritual needs of the patient and family.  Responsible for initiating the appropriate intervention and support for the patient and family upon admission to VITAS and provides a continuously appropriate, comprehensive and responsive plan of care.

• Qualified candidates must be currently licensed as a Registered Nurse in the state of California.
• Minimum one (1) year experience in acute or sub-acute care required with hospice or home health experience preferred.
• The full-time RN Case Manager will perform patient visits on the peninsula of San Francisco Bay in San Mateo County and qualified candidates will reside in San Mateo County and must be residents between Sunnyvale and Redwood City in order to respond to patient/family needs in a timely manner.
• Must have reliable transportation with appropriate insurance coverage.
• This is a full-time position working M-F 8a-5p and rotate on-call responsibilities at night and on weekends with other team members.

• Completion of course study as required, acquiring RN state licensure.
• ADN required.
