The Home Care RN performs the initial evaluation visit and regularly reevaluates the patient's nursing needs. The RN will also be responsible for: - Initiating the plan of care and necessary revisions
- Performing services in accordance with the plan of care
- Preparing clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner
- Counseling the patient and family/significant others in meeting nursing and related needs
- Processing orders and notifies physician of patient needs and changes in condition
- Completing certification/recertification orders and discharge summaries
- Referring to Physical Therapists, Speech Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills

- Registered Nurse
- Active MA licensure
- 1+ years of home care experience
- Oasis experience (computer version)
- Excellent communication skills
- Strong attention to detail
