As the RN Case Manager, you will:

• Tend to geriatric clients
• Provide implementation and evaluation of individual patient plan of care
• Teach and counsel both patients and family
• Provide care that conforms with general medical policies
• Schedule work load for maximum efficiency

• 3+ years geriatric experience
• RN Certification
• Home care experience
• Strong telephonic/administrative skills
• Excellent computer skills
• Great communication skills
• Desire to transition clinical skills into case management
