RN Case Manager

A medical group in Boston is in the market for a nursing professional to join their staff as a Case Manager. The RN Case Manager will be responsible for developing a Plan Of Care that corresponds with the physician's orders through assessment of patient's needs, condition, environment, and consultation with other health team members at time of initial visit. Th nursing professional will also be responsible for updating plan of care regularly as necessary and documenting this care following Agency procedures.

The RN Case Manager is responsible for:

• Delivery of competent, quality patient care in the home
• Maintaining an ongoing responsibility for assigned caseload
• Implementation and evaluation of individual patient plan of care
• Providing teaching and counsels both patient and family
• Providing care that conforms with general medical policies
• Scheduling work load for maximum efficiency
• Regularly conducting Case Care Conferences
• Abiding by and promoting Agency administrative policies
• Maintaining a continuing knowledge of competencies related to the Nursing profession by participating in formal education programs, conference, workshops and professional organizations

• 2+ years of case management experience
• Active MA license
• Microsoft Office proficiency
• Strong attention to detail
