Under the general direction of the supervising physician and/or Medical Director, the nursing professional will be responsible for coordinating the comprehensive health care needs of the specific patient population and providing appropriate direct patient care through assessment, diagnosis, and treatment of specific conditions. | |

The Physician Assistant/Nurse Practitioner is responsible for: - Providing primary care services to individuals and families across the lifespan.
- Practicing within the collaborative model with physicians, nurse practitioners and clinical psychologists
- Providing and managing direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population
- Assessing patient's physical and psychosocial health/illness status through history taking and physical examination
- Diagnosing, managing, and appropriately referring patients with acute, episodic, or chronic illness according to proper protocols
- Prescribing pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions
- Referring patients to specialists and to relevant patient care components as appropriate
- Directing and coordinating the patient care activities of nursing and support staff
- Following established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standard
- Participating in specified health promotion, education and/or prevention programs.
- Ordering appropriate laboratory and diagnostic tests according to established guidelines
- Facilitating and coordinating patient care referrals to specialists and outside agencies
- Developing and providing educational materials and/or programs to patients, families, and other health care professionals at the Center

- 3+ years of nursing experience
- Massachusetts Licensed Physician Assistant or Nurse Practitioner
- Effective verbal and written communication skills
- Knowledge of legal and ethical standards for the delivery of medical care
- Knowledge of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosages and instruct patients in correct usage
- Knowledge of community medical diagnostic and patient care services in area of medical expertise
- Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise
