Registered Nurse (RN) - Intake

A home health care agency has an opening for a Registered Nurse (RN)- Intake. In this role, the qualified professional selected for this role will work onsite at the hospital admitting patients into the home care program.

As the RN- Intake, you will:

• Work onsite at  Hospital admitting patients into home care program
• Evaluate patients referred to home care from the hospital’s Emergency Department (ED)
• Plan, write and coordinating referrals for home care services
• Collaborate with the hospital’s ED case managers, discharge planning personnel, utilization review and insurance case managers to facilitate safe discharge
• Complete intake referral information including intake data, essential background information, insurance verification, hospital course and plan of care
• Maintain relationships with hospital personnel, providing information, education and marketing agency services

• RN
• 2+ years of home care experience
• 2+ years of experience with CHHA or Lombardi, and OASIS
• Medicare regulation knowledge
• Excellent communication skills
• Detail oriented
