The Care Coordinator Supervisor maintains a central role in assessment of health home patients and in receiving and analyzing patient acuity and provider information via claims date. The qualified candidate will be responsible for ensuring that of these vulnerable patients receive timely assessments, plans of care and care coordination and ensures that patients receive optimal care, including: acute illness management , chronic disease management, and preventive care across multiple health settings and multiple providers.

 

The qualified candidate will have a degree as a Nurse Practitioner & NYS license as a Nurse Practitioner; 5 years of experience managing chronic diseases, working with Mental Health patients, & supervision of staff.
