Position Summary:


Provides patient care as part of a multidisciplinary team based on national standards and utilizing the nursing process.  (Position requires a MINIMUM of one year experience.)

 

Position Expectations & Responsibilities:

 

  • Expected to demonstrate a good working knowledge of regulations for patient care and documentation.
  • Expected to have basic knowledge of office procedures.
  • Expected to have basic knowledge of Horizon Homecare system.
  • Expected to have intermediate mathematical skills.
  • Expected to have the ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or governmental regulations.
  • Expected to demonstrate a good working knowledge of CMS/AHCA/OSHA/The Joint Commission regulations for patient care and documentation.
  • Expected to recommend solutions for complex problems.
  • Expected to initiate and promote a multidisciplinary approach to patient care.
  • Expected to recognize the influence of beliefs and cultures on behaviors and accept strengths and limitations in others.
  • Expected to exhibit adaptability, flexibility, self control and maturity in work and behavior
  • Expected to exhibit calm responsiveness to emergency or stressful situations.
  • Expected to recognize situations which require notification of a supervisor.
  • Expected to possess diplomatic assertiveness in the management of the patient’s home care needs.

Field Medicare RN

  • Expected to deliver skilled, professional and age-appropriate intermittent patient visits at home.
  • Expected to coordinate total plan of care as directed by physician and case conferences to assure continuity of care.
  • Expected to develop, prepare and maintain individualized patient care progress records with accuracy, timeliness and according to policies.
  • Expected to reassess patient at each visit and revise plan of care as appropriate.
  • Expected to recommend solutions for complex problems.
  • Expected to complete and maintain all required documentation.
  • Expected to supervise and teach the home health aide.
  • Expected to involve patient/family/significant other in total plan of care.
  • Expected to report changes to the physician as needed.
  • Expected to communicate plan of care changes to appropriate personnel.
  • Expected to effectively present information and respond to questions from groups of managers, clients, customers and the general public.
  • Expected to communicate verbally and in legible writing in the English language in a manner that can be understood.
  • Expected to attend and participate in department staff meetings as well as participate in hospital meetings, forums and seminars.

Office Medicare RN

  • Expected to audit records at appropriate intervals.
  • Expected to take referrals from hospitals, physician’s offices, care facilities, etc. and screen for appropriateness for home care.
  • Expected to perform pre-discharge assessments of patients to determine eligibility for home care under Medicare guidelines.
  • Expected to obtain private insurance verification for benefits and authorization.
  • Expected to assist with development of PI programs and analysis of data collected to improve patient outcomes.
  • Expected to identify patterns of potential problems through data collected from audit activities and special projects and report them as appropriate.
  • Expected to assist in orientation of field staff regarding documentation guidelines.
  • Expected to record and report findings from audit records to appropriate personnel.
  • Expected to initiate communication, written and/or verbal, to field staff, according to audit findings and assist in proper documentation as needed.
  • Expected to communicate plan of care changes to appropriate personnel.
  • Expected to effectively present information and respond to questions from groups of managers, clients, customers and the general public.
  • Expected to communicate verbally and in legible writing in the English language in a manner that can be understood.
  • Expected to attend and participate in department staff meetings as well as participate in hospital meetings, forums and seminars.

Minimum Education and/or Experience: 

  • Diploma from an accredited school of nursing or Associate's degree
  • MINIMUM of One year of experience as a Registered Nurse is REQUIRED

Licensure/Regulation/Certification: 

  • BLS certification from the American Heart Association
  • Current Florida Registered Nurse license in good standing
