Job Description:
Evaluates patients referred for home health services, Hospice and/or Palliative Care. Manages and facilitates the transition of patients in hospice or palliative care or onto the home care program in accordance with applicable law, regulations and agency policies and procedures.

Job Requirements:
Associates in Nursing; Baccalaureate Degree preferred. Minimum of two years experience in an acute, Medical-Surgical nursing, Community Health Hospice or Oncology as well as CHHA/LTHHCP experience preferred. Licensed with current Registration to practice as Registered Professional Nurse in NYS; Current CPR Certification. Word and Excel.
