Description

St. Rose Dominican Hospitals Case Management Department provides Case Management, Utilization Review and Social services to an all encompassing patient population, inclusive of a full service line. The goal of the continuity of care function is to define, shape, and sequence the patient’s clinical care and activities along the hospital admission and care continuum through to discharge. The Case Manager bases care recommendations on information gathered from many sources and team members, about patients’ needs and insures the patients are placed in the appropriate level of care through the application of Interqual criteria.

Job Summary:
Under the general direction of the Case Management Director, is responsible for assisting in the ongoing development, formalization and administration of the Case Management Department. This position assists in the management of staff productivity, personnel issue and performance. It assist in providing an organized patient focused system that coordinates the delivery of quality healthcare services across a continuum of care by continually monitoring, evaluating, and modifying the treatment plan to achieve optimal patient outcomes. This system emphasizes patient and customer satisfaction and balances individually identified patient and family needs with the efficacious use of resources while following the rules and regulations to maintain hospital licensure.

Experience
Minimum — Five years acute hospital clinical experience as a RN. Three years of hospital Case Management required. Past Supervisory experience required. Critical thinking and sound clinical assessment skills are essential.

Education
Minimum - Graduate of an accredited nursing program.

Special Skills
Ability to work independently, read and analyze clinical data, communicate effectively verbally, on the telephone and in writing. Must be able to maintain patient and family confidentiality. Must be able to cope with a continuously changing environment by demonstrating flexibility, above average customer service skills and required. Working knowledge of InterQual Guidelines.

Licensure
Required: RN = Current license as a Registered Nurse in the State of Nevada.
CT CPRBLS = Basic Life Support
Preferred: Case Management Certification. CT CCM
