Job Description:
Responsible for complete care management of members needs and coordinates services with providers. Collaborates with Medical Doctors in delivering care to patients. Responsible for adequately addressing the progression of the patient's illness and the corresponding home care services required. Coordinates provisions of all ancillary services as well as health care services not included within the HomeFirst benefit package. Travel to patient homes is required. Job Requirements:
Associates/Diploma from an accredited school of Nursing; BSN preferred. Basic knowledge of Microsoft Word, Excel and e-mail skills. RN licensed to practice in NYS.
