Working with minimal supervision the Breast Health Navigator is responsible for the assessment, planning, implementation and evaluation of the Breast Health Center Services. This includes, but is not limited to the coordination of care provided to the client as well as serving as a liaison between the physician, healthcare staff and other involved parties. The Breast Navigator will also report outcomes and participate in quality initiatives. Additionally, the Breast Health Navigator may assist in minor surgical procedures, coordinate follow-up care, provide patient education and assist in research activities. This position is Grant-Funded.
Qualifications
Requirements include a Bachelor's degree with advanced nursing practice and healthcare coordination preferred.
Current Sate of Connecticut nursing license is required.
At least 3 years experience in breast health services is preferred but not mandatory. OCN certification is preferred but not required. A broad knowledge of community health concepts and serices and the ability to organize and oversee the development of the patient care team is essential.
