Provides staff development guidance to assist staff providing patient education based on established protocols or guidelines.
Provides consultative services and resources to nursing and medical staff for patient teaching problems.
Monitors, evaluates and revises patient education program for specific population related to clinical expertise.
Assesses the learning needs of a specific patient population for new teaching programs and materials. Assesses patient specific educational needs and level of learning, then formulates a teaching plan.
Teaches patients/family individually and in class setting on specified conditions.
Utilizes the adult teaching and learning principles in the educational process.
Evaluates the effectiveness of each new program produced.
Provides consultative services to nursing and medical staff for patient teaching problems.
Participates in multidisciplinary team conferences.
Consults with other allied health personnel.
Demonstrates competence to adapt work and customer service assistance methods to accommodate the unique physical, psychosocial, cultural, safety and other developmental needs of patients served by the department.BSN with certification in specialty or demonstrate competency in area of expertise; Current, valid Texas licensure
Two (2) years clinical experience in area of practice; One (1) year patient education experience
Travel between HCHD facilities and in community
Bilingual skiils (English/Spanish) required for CHP Educators
